Background : Lipoid pneumonia is a rare disease and usually reported as sporadic cases. Exogenous lipoid more common form of pneumonia. Untypical characteristics makes incidence rate this cases are often imprecise. The diagnosis made from history lipid containing material aspiration, radiologic imaging, histopathological examination. There no treatment choice because rarely made.
 Case Report We report one case at Dr. Saiful Anwar hospital, 35 years old man with accidental kerosene ingestion aspiration. Based on examination the patient, laboratory results, radiological BAL fluid analysis cytology, exogenous was made. Our combination antibiotic, corticosteroid to manage patient. After 2 weeks treatment, there were improvements clinical imaging.
 Conclusion: disease. Using corticosteroid, through bronchoscopy can be therapeutic option that provides improvement.
